Birth Certificates

A birth certificate is a confidential document; therefore, only the individual, the biological or adoptive parents of the individual, the biological or adoptive grandparents of the individual, the guardian or temporary guardian of the individual, a legal spouse, next of kin, or personal representative of the individual’s estate, or an adult child of the individual can request and obtain the birth certificate.

A legal guardian of a minor or incapacitated adult must present proof of legal relationship to a child or incapacitated adult at the time a birth certificate is requested. Acceptable forms of proof include certified copies of court orders regarding custody or Letters of Guardianship that have been certified within one year of the date presented.

If you were born anywhere in the State of Georgia, you may request a certified copy of your birth certificate from the Peach County Probate Court.

To obtain a birth certificate you must present a Georgia Driver's license or State of Georgia Identification Card unexpired or expired for not more than one year, State of Georgia Weapons Carry License, unexpired Driver's License or Identification Card issued by another U.S. State, jurisdiction or territory, unexpired U.S. Passport, unexpired Foreign Passport, U.S. Military Identification, Military Dependent Identification, Veteran's Identification, unexpired Consulate Card, Transportation ID, Debit Card with Picture and Employer ID Card.

Requests by mail must include the following information: Full name of the child, mother's maiden name, father's name, and child's date of birth. You must include your check or money order made payable to Peach County Probate Court, a self-addressed stamped envelope back to yourself, and a clear photo copy of your picture I.D.

The cost of obtaining a Georgia birth certificate is $25 per person for the first certified copy and $5 per person for each additional certified copy.

The Probate Court accepts cash, credit card (Visa, Discover, MasterCard), personal check or money order payable to the Peach County Probate Court. The Probate Court will not third party checks.
